A. Students upto Class (Grade) 12^th:

Three options to establish the student’s identity and proof of class (grade) for which student performed the activities and won the prize

• Primary Document: School Photo ID Card

  Additional Document: NONE

Note: School photo ID card must clearly represent the Class (Grade) and session (Year) attended by the student.

OR

• Primary Document: Govt Issued Photo ID Card of Student

  Additional Document: School ID Card (without photo) or a Verification Letter from the School

Note: School photo ID card must clearly represent the Class (Grade) and session (Year) attended by the student.

OR

• Primary Document: Birth Certificate of student showing parents name

  Additional Document: (both required)

(i) Submit either Father’s or Mother’s self attested govt issued photo ID card, and

(ii) Submit signed letter from the parent whose self attested govt issued photo ID card has been submitted stating that their child (the student full name and date of birth) is a student of class (grade) for which the child has performed the activities and won the prize

Note: Name on parent’s govt issued photo ID must match the name of parent mentioned in the Birth Certificate of the student

B. Students above 18 years of age:

Two options to establish student’s identity;

• Primary Document: College or University Photo ID Card

  Additional Document: NONE

OR

• Primary Document: Govt Issued Photo ID Card of Student

  Additional Document: NONE

Students who are winners and have received ‘Gift from Parent’ points:

If a student has used points as ‘Gift from Parent’ to win the prizes, then the student must also submit the following document related to Mother or Father or both from whom they have received ‘Gift from Parent’ points.

• Primary Document: Birth Certificate of student showing parents name

  Additional Document:

(i) Mother’s Govt issued photo ID card (if ‘Gift Points’ received from mother)

(ii) Father’s Govt issued photo ID card (if ‘Gift Points’ received from father)

Note: Name on parent’s govt issued photo ID must match the name of parent mentioned in the Birth Certificate of the student

C. Parents, Faculty and Non Students above 18 years of age:

One option to establish identity;

• Primary Document: Govt issued Photo ID Card
